After completing the GLP-1 Eligibility Intake Form, you will be prompted to enter your payment information. Please note that you will not be charged at this stage. We will securely hold your payment information until a decision is made on your case. After, your card will be charged accordingly.
Upon successful payment processing, you will receive a confirmation email along with a payment receipt for your records.
